Car accident report form for IA?

I got into a car accident with my girlfriends car in IA on Fri morning. the other guys was 100% at fault and has admitted it and apologized numerous times. We didnt call the cops at the seen because we were both a little rushed. He didnt want to report

If you report it to DOT then rates will go up. Insurance companies use your DOT record among other things to determine rates. If you do not report the accident however you may have a more difficult time should he choose not to pay you. If he was at fault
